The following form is the USA Defenders of Greyhounds standard adoption application. When you submit the form, the contents will be emailed to a USA D.O.G. representitive. The entire form must be filled out or you will get an error message asking you to fill in the blank areas. If that field doesn't apply to you, please type "NA" in that space. Please understand that it may take 4 to 6 days for someone to contact you about your application, as we are an all volunteer organization. Many times we will get back to you sooner than that, but we would ask you to please be patient. While we strive to contact prospective families as quickly as possible, please remember that we are an all-volunteer organization, and it may take 4 to 6 weeks for us to reply.
